Single mother is in need of a kitchen table that seats 8, fathers are Mother just started her current job not too long ago and is currently on leave due to medical reasons.

The items are needed for a 13-year-old teenage boy and a 9-year-old boy. These items will provide a safe and comfortable sleep environment. The children share the same room and are both sleeping on beds that are broken and unsafe.

Erica Hugh is in need of help in order to provide safe sleep and to keep the children from harm. She is living in a home that is in need of the following items needed to ensure children safety. The home has limited heating.

• full size bed with a comforter set.

Grandmother is caring for child as mother is in and out of the home. Mother is sole provider for the home and grandchild. Child is currently sleeping with grandmother who is in need of a toddler bed.

We have some students with speocal needs that are in meed of school uniforms.

This family is a single mother with 3 children. 13 yr old has Type 1 diabetes. The youngest just turned 1 yrs old. Mother has financial issues and sometimes cannot afford medical supplies that Medicaid does not cover. Like testing strips. Mother just returned to work from non-paid maternity leave and had some medical issues of her own. Food stamps does not cover food through the whole month. 13 yr old female needs med size underwear, size 8 shoes, size 2 pants (teen) med and med shirts. 9 yr old male needs, size med 9-10 pants and shirts, underwear and size 5 boy's shoes. Baby boy wears 18 months clothing and size 3 infant shoes. Size 4-5 diapers. Enfamil Gentlease in purple can.

Requesting assistance for a 13 year old child that was able to returned home to her mother after being in foster care for 4 years. The 13 year old child is needing a bed to sleep in comfortably. Thank you very much

I have a mother of three boys who is currently pregnant with her fourth child, whom is a girl. She is due in May, but is more than likely going to have the baby in April. She was in a domestic violence situation and has since left her partner and is currently in a shelter. He is not helping her financially at all. Two of the children are his, and the one that she is pregnant with. Due to the severe domestic violence, she was isolated from her family and friends. She is starting to mend relationships, but she does not have any current support that can help her obtain the items in this request. The shelter has attempted to help her as much as they can get the items she needs, but she is still lacking a bassinette and a car seat for the new baby. Any help is greatly appreciated.

Household consists of 30 YO mom and 4 month old boy. Concerns came in due to drug concerns. Mom recently moved from Tyler and is living by herself. The Department is trying to help connect her with support for substance treatment. Mom has no furniture whatsoever in her residence and is in need of a bed. She is using an air mattress for her and her newborn. The Department is trying to help her with a pack-n-play and basic items for her child. We are requesting furniture and bus passes that could help her with her future appointments.

Hello. I have a mom who has worked very hard to get her children back into her care. We are at our final steps of the case and are in need of some assistance. The mom is in the process of trying to reunify with her 3 girls ages 4, 2, & 1. We would like to start having visits to transition to a full return of the kids. The mother in this case needs car seats for her children to transport them.

Mother contacted the Social Work Hub right before Xmas asking for bunk beds for her children, the one they had picked up from the trash was infested with bugs. She then called back and said she was able to find some mattresses from a friend, she contacted me today to say they were also infected. She send me pictures of the bugs and she was not able to send the kids to school yesterday and today because they have bug bites all over their bodies.

This request is for a child that resides with his mother who had been homeless. I became aware of this child’s need when he asked if we could help him with $200 for a holding fee since they had been accepted for housing and was granted a prorated rent of $133.I have connected with the family and have helped to identify that this family is in need of and can benefit from the following priority items: *$200 holding fee I am aware that this family has faced many adversities and challenges upon moving here to Texas after living with grandmother in Louisiana. The mother of this child was very young when having him and are now at a point in their lives where they are getting on their feet.
